*Vets who accept faxed copies of Muffin's certificate. Make an appointment with vet first, then charge certificate  and where it says comments, write the vets office and fax #,  appointment date/time and the pets name. Shots/antibiotics/pain management NOT included with certificate.  Ask about additional fees.
If your pet is a PURE BREED, check with the vet before buying the certificate.  Some vets do not accept the certificate for a PURE BREED.
Pet owners: Please do not fax certificate to vets office.
