* Vets who accept faxed copies of Muffin's certificate.  Make an appointment with the vet first, then charge certificate.  If ordering online,   there is a  comment box.  After  charging certificate, write FAX TO: the name of the vets office, appointment, date, time and the pets name.   Shots, antibiotics, pain management, blood tests for older pets, retained testicles, pyometra, stool testing, in heat, abortions, declawing, etc are NOT included with certificate.  Ask about additional charges and other medical procedures before making appointmentThe doctor has the right to refuse services and Pure Breed pets.  
Do not fax certificate to vet's office!

CHECK HOSPITAL POLICIES FOR LEX VET

Pre-Anesthetic Blood work to be done by our Veterinarian.  All patients are required to have pre-anesthetic blood work prior to anesthesia being administered for all procedures. Anesthesia poses a risk regardless of species, breed, age or sex. This blood work evaluates the kidneys, liver, blood sugar, protein levels, evidence of a any infections (white cell count), anemia (red blood cell count), and the ability for the blood to clot (platelet count).

Proof of current vaccinations and tests listed below: If pet is not current on anything listed, they can update it for you. Inquire for further details when making appointment. 
 
Dogs - DHPP, Rabies, Bordetella (every 6 months), Fecal/Giardia (negative results) Heartworm negative results

Cats – FVRCP, Rabies, Fecal / Giardia (negative results) and Feline Leukemia and FIV (negative results).
